How Not to Implement Lean: a Story About Failure

Efficiency was low, problems were persistent and did not get solved, or returned at a later date after being ‘solved’.

We have attempted Lean a number of times over the last 15 years. While the attempts did make a difference, this presentation will look at the key areas where it didn’t!

We started a new Operational Excellence program at the beginning of 2025. This is still in progress and we are not claiming success yet.

  • Openness and transparency is required to identify failure
  • The value of identifying both the good and the bad in any initiative, whether it works well, or doesn’t…
  • The importance of resilience and perseverance
  • Examples of attempts at Lean and why they failed, but also what we learned
  • Overview of our current OpEx program, highlighting where we have applied learnings from previous attempts
